What is the prognosis for nasopharyngeal carcinoma?
Because NPC is highly radiosensitive, stage I disease has five-year survival commonly above 90%, and modern IMRT series report good control for locoregionally advanced disease. Prognosis worsens with bulky nodal disease, skull-base invasion, high pre-treatment EBV DNA, detectable EBV DNA after treatment and distant metastasis. Over 90% of recurrences occur within five years, so structured follow-up matters.
In detail
Published survival figures are population averages from registry data. Your own outlook depends on stage, HPV or EBV status, margins, nodal involvement, general health, smoking status during treatment and response to the first treatment.
Most recurrences occur within the first two to five years. Follow-up visits are frequent at first (every one to three months in year one) and become annual after year five, in line with NCCN survivorship guidance.
