Immunotherapy and Targeted Therapy in Head and Neck Cancer

Where PD-1 inhibitors, EGFR antibodies and mutation-directed drugs fit, what PD-L1 CPS means, and what international patients must verify before continuing treatment abroad.

Quick answer

Immunotherapy is mainly used for recurrent or metastatic head and neck squamous cell carcinoma: pembrolizumab (alone or with chemotherapy in PD-L1-expressing disease) and nivolumab or pembrolizumab after platinum failure. Cetuximab targets EGFR, nimotuzumab and PD-1 inhibitors such as toripalimab feature in Chinese nasopharyngeal protocols, and BRAF/RET/NTRK inhibitors are used for specific mutations.

Where do these drugs fit in the treatment pathway?

  • First-line recurrent/metastatic HNSCC: pembrolizumab alone for PD-L1-positive disease or with platinum/5-FU chemotherapy; PD-L1 combined positive score (CPS) helps select therapy.
  • After platinum failure: nivolumab or pembrolizumab improve survival compared with standard single-agent therapy in this setting.
  • Locally advanced disease: perioperative immunotherapy is an evolving area (for example pembrolizumab in resectable locally advanced HNSCC in recent guideline updates) and is best delivered within trials or defined protocols.
  • Nasopharyngeal carcinoma: PD-1 inhibitors (toripalimab, camrelizumab and others) and the EGFR antibody nimotuzumab appear in Chinese guideline-based protocols; availability differs by country.
  • Thyroid cancer: multikinase inhibitors and mutation-specific BRAF/MEK, RET or NTRK inhibitors for progressive disease; radioactive iodine remains the main systemic treatment for differentiated cancer.

What tests decide whether a drug is appropriate?

PD-L1 immunohistochemistry with a combined positive score (CPS) is the most widely used immunotherapy biomarker in HNSCC; a higher CPS is associated with greater likelihood of response, though benefit is not limited exclusively to high scores. HPV/p16 and EBV status, MSI/MMR and tumour mutational burden, and mutation profiling (BRAF, RET, NTRK, HER2 in salivary gland cancers) can open other options.

Biomarker testing should be done on a representative tumour block before systemic therapy starts. Bring the block or unstained slides if you are seeking a second opinion in another country.

What is different about immunotherapy side effects?

Immune checkpoint inhibitors can cause inflammation in any organ — thyroiditis, pneumonitis, colitis, hepatitis, dermatitis, hypophysitis and, rarely, myocarditis or neurological syndromes. They may appear weeks to months after the last dose and require corticosteroids or endocrine replacement. This is different from the predictable, cycle-related toxicity of chemotherapy and is the main reason a written continuation plan is essential when travelling.

What international patients must verify in writing

  • Exact generic and brand name, dose, cycle interval and last infusion date of your current regimen.
  • Whether that exact product is stocked at the Chinese centre, and the per-cycle price.
  • Recent imaging, response assessment and any immune-related adverse-event history.
  • Who will manage immune-related toxicity, and which tests (thyroid function, liver enzymes, cortisol, chest imaging) will be monitored.
  • A discharge summary and cycle calendar in English for your oncologist at home.

Frequently asked questions

Is immunotherapy a replacement for surgery or radiotherapy?

For curable localised disease, no. Surgery and radiotherapy remain the standards. Immunotherapy is established in recurrent/metastatic disease and is being studied in the perioperative and locally advanced settings, ideally within clinical trials.

What does PD-L1 CPS ≥1 mean on my report?

CPS is the number of PD-L1-staining cells (tumour cells, lymphocytes and macrophages) divided by the total number of viable tumour cells, multiplied by 100. CPS ≥1 indicates a positive score and is used to guide first-line pembrolizumab use; higher scores generally correlate with greater benefit.

Are Chinese immunotherapy drugs the same as those used in the US or Europe?

Not always. Some agents (for example toripalimab, camrelizumab, sintilimab, tislelizumab) are widely used in China but may not be approved or stocked elsewhere. Check the exact generic name, indication and approval status in your home country before agreeing to a switch.

Can I combine immunotherapy with traditional herbal medicines?

Tell your oncologist about every supplement or herbal product. Some interact with drug metabolism, affect liver or kidney function, or muddy the interpretation of immune-related side effects. Do not start new products during immunotherapy without checking.
